HMIS Data Standards changes have been implemented as of October 1st, 2017.
Project Start Date and Permanent Housing Project Workflow Changes
The biggest of the changes is the concept of a project start date and a housing move in date.
Permanent housing projects will be most impacted by this change. A Permanent Housing, Permanent Supportive Housing and Rapid Rehousing project will now create an Entry when the client is admitted and determined eligible.
After Entry, once the client has moved into their housing unit, the Permanent Housing project will document a Housing Move-in Date. Housing Move-in Date is documented in an interim review of an "Update: Housing Move-in Date" type.
An example of this new workflow for PH, PSH and RRH is as follows:
Gender Picklist Options
Gender options have changed as well.
As a reminder, Agencies record the self-reported gender of each client served. Staff observations should NOT be used to collect information on gender.
Gender does not have to match legal documents and clients may not be asked about medical history to determine how they identify.
A good question to ask your client so they can self report would be “Which of these genders best describes how you identify?”
Questions Removed from the Disability Subassessment
Questions related to PATH and documentation on the disability subassessment will be removed.
Health Questions Added
Four questions related to the client’s health will be added after the health insurance subassessment:
- RIN (Recipient Identification Number)
- What health plan are you enrolled in?
- Have you visited your primary care physician within the past 6 months?
- Where have you gone most often to seek medical care in the past 12 months?
